1. Print and sign the Affiliate Agreement

2. Print and sign the W-9 Form

3. Fax page 9 (signed) of the Agreement and the W-9 form to (800) 906-1654.

4. Fill out the form below.

5. Sales resulting in an Annual or Monthly Membership: 35% of Net Revenue per Month for the first year of the Client's membership, and 25% the following year. Commissions paid monthly.
